Residency Verification Documentation
(Required documents to register)
Please gather the original copies of the following required documents to bring to your registration appointment. The documents will be copied and/or scanned for our files and the original copies will be returned to you.
You are encouraged to black out any account and/or social security numbers on the documents. All documents must be current and show your name and address.
You must provide all four (4) of the following documents that are dated within the last 30 days.
You need one document for parent/guardian identification, one document from “Category A”, AND two documents from “Category B” (see below)

** SPECIAL NOTE:
Families who have a complex guardianship situation or residency matter will need to contact regparenthelp@district130.org to schedule an appointment to discuss the situation.
Families Living with a Cook County School District 130 Resident: If the parent/guardian and family are residing with another family within the district, both the person who owns or rents the residence AND the parent/guardian must complete a Student Residency Affidavit form. The person who owns or rents the residence is required to be present or have the Homeowner Affidavit notarized and they must provide copies of their Driver’s License/State ID with the current address plus a current document under Category A. The parent guardian must be present and provide their photo identification and three (3) documents of Category B.
CCSD130 Parent Guardian Affidavit
